4650 artifact problems

hey guys.....about 4 months ago, i purchased sapphire ati radeon hd 4650 512mb gddr3 with hdmi pcie 2 x16 from newegg, ever since, whenever i open up a game and start playing, i experience heavy artifacts flashing everywhere, whenever i play a video on full screen, i either crash or artifact till the driver crashes (same for gaming) so, i decided to upgrade my power supply thinking that was the cause so i purhcased a diablotek 450 watt psu (original liteon 300watt), which still did not correct any problems what should i do?
my specs: amd athlon 64 x2 6000+ (no overclock)
sapphire ati radeon hd 4650 (no overclock)
gigabyte ga-ma78gm-us2h
3gb samsung ddr2 pc-5300
diablotek 450 watt psu
toshiba dvd supermulti drive
old sony case
seagate 80gb ide 5400rpm
seagate 500gb baracuda 7200rpm sata
running windows 7 rtm x64 (currently, tried this card on xp and vista yielding the same results)
any help would be appreciated

Well if you have had the same results in multiple computers I would have to say you got a bad card. I would contact saphire and rma it.
