Power Supply Death, But Why?

I recently put together a PC for a friend of a friend. He ordered the an OCZ ModXStream 600W, due to build quality and price. I had the PC up and running fine. My friend brought his PC over and I unplugged the original one to work on his. Go to plug his back in and press the power button, nothing, nada, zip, zilch. I get out my Coolmax PSU Tester and the screen flashes for a second then dies.
Attached (sorry it's Hotfile but it includes the OCZ and a working power supply) are some videos. They were converted to DivX format and compressed with WinRAR if you are having trouble opening the files.

When I went to unplug this machine the power cord was secured good. I removed it with some force. Did I possibly break something removing the power cord? I've never seen this happen and I'm at a loss.
I've had DOA power supplies, one that blew and several that just went dead. This is not something I've encountered.
