Audio editing question

I know that there are some good apps out there like Adobe Audition. What I don't know is how to smoothly blend the ends of two clips together to eliminate the transition jerkyness. TIE Fighter never had a soundtrack, rather it used iMuse technology to blend a soundtrack dynamically. However, I have obtained from my copy of TIE Fighter the MIDIs of the game. Unfortunatly, I don't know how to piece them together so that the transitions are nonexistant. So I need an app/how to that would tell me how to match the pitches of the very last note of one clip to the first note of another OR blend the end of one clip with the beginning of another. Crossfading doesn't work, I tried that.
