Problems with ATI SAPPHIRE HD 3850 1GB

Wen i start my PC the screen is black or in lines and i think it may be connected to the ATI CCC because it shoal's that the graphic card is HD3800 series not HD3850(the attachments show this)
Motherboard:Gigabyte GA-EP45-EXTREME
Processor:Intel Core™ 2 Duo E8500
Can any one help me

It showing as 3800 is normal. The problem shouldn't be there.
Do you get the same problems when no AMD/ATI display drivers are installed?

No after the installation of the drivers

Have you tried a different version of the drivers?
What OS are you running?`

yes i changed 3 version CCC 8.1(original from the CD) CCC 8.7 and now I'm whit 8.8
the operating System is Windows XP Professional x64
