Why would this happen? I plug in my printer (drivers ARE installed) or a USB HDD (plug and play, yes the PnP service is running) and I get the "USB Device Not Recognized" error message. Oddly, my USB mouse works fine and always shows up in BIOS (USB Devices - 1 Mouse). I have a K8N Diamond Plus with WinXP Pro SP2
My Device Manager is messed up, it has "Standard Enhanced PCI to USB Host Controller," "Standard OpenHCD USB Host Controller" and two "USB Root Hubs"

Nope, no go. There are two USB Root Hubs in my Device Manager. One says "10 ports, 9 availible" (mouse plugged in) and the other says "10 ports, 10 ports availible." I have, in reality, six ports. No, the number of ports cannot be set in the BIOS. It is either USB Controller On or USB Controller Off. Yes I have tried "toggling" it. No effect. Unplugging the two ports on top of the case does NOT help. For some reason, plugging in the mouse works no matter what port I use.

I ran into this problem while installing my digital camera.
It may not be the drivers nor the device plugged into the USB port.
Windows may have the USB port turned off. There is a way to turn it back on in the settings. It's been too long for me to remember how to do it.

I turned on the USB ports in Windows. I think I fried the physical ports (not the mobo ports)....
